Yale New Haven Health’s Know Your Numbers (KYN) Plus program gives you the power to make decisions that impact your health.

With the COVID-19 pandemic shining a light on the importance of well-being, a biometric screening that provides a snapshot of your physical health (your “numbers”) is an important piece of knowing where you stand. Schedule your screening today to get a jump start on your health (details follow).

“The KYN Plus program doesn’t stop there,” said Melissa Turner, senior vice president and chief human resources officer, YNHHS. “As part of Thrive, the hub for all YNHHS employee well-being resources, KYN Plus includes an expanded list of activities, extending into areas such as emotional, physical and financial wellness, to help bring your health and well-being full circle.”

While employees may use the many Thrive resources year-round to enhance their overall well-being, the KYN Plus component of Thrive is now underway and runs through Sept. 15.

To participate:

  1. Enroll in the Virgin Pulse employee well-being portal (if you haven’t done so already).
  2. Receive a biometric (health) screening – schedule it on the Virgin Pulse portal.
  3. Complete Thrive / KYN Plus personal choice activities. 

All employees are encouraged to participate in the KYN Plus program to enhance their well-being. YNHHS benefits-eligible employees hired on or before Aug. 14, 2022, can earn up to $500 credit towards their 2023 YNHHS Medical Plan premium by participating in the program. 

Step 1: Enroll in the Virgin Pulse employee well-being portal
Employees who choose to participate in KYN Plus must enroll in the portal. (Employees who have already enrolled in the portal do not need to re-enroll — log-in using Single Sign-On.)

Access the portal from the link in this article. (There are also links on Infor and the Resources section on the intranet.) YNHHS benefits-eligible employees must also complete Steps 2 and 3 to earn up to $500 credit toward their 2023 YNHHS Medical Plan premium. 

Step 2: Receive a biometric (health) screening
Biometric screenings are brief, in-person health assessments that provide a picture of your overall health and alert you to any possible health risks. All employees are encouraged to receive a biometric screening for your own benefit to “Know Your Numbers.”

Onsite biometric screenings are well underway at all delivery networks by appointment. Screenings can be scheduled using the Virgin Pulse portal – the link is also on the Action Dashboard on Infor.

If preferred, benefits-eligible employees may receive a biometric screening from their healthcare provider or at a CVS Caremark location and upload documentation confirming the screening to the Virgin Pulse portal. To receive KYN credit for the screening, employees must upload documentation from the off-site screening to the Virgin Pulse portal by Sept. 1, 2022, to ensure it is received by the deadline. 

Completing the biometric screening earns $200 credit (toward the possible $500 total credit). Employees must complete the biometric screening in order to earn additional credit through KYN Plus.

Biometric (health) screenings

Biometric screenings provide the following data:

  • Total cholesterol, HDL (good cholesterol), LDL (bad cholesterol), triglycerides and blood glucose
  • Blood pressure
  • Height, weight and body mass index (BMI)
  • Waist and hip measurements
  • A1C, for employees’ with high fasting or non-fasting blood glucose

Step 3: Participate in personal choice activities
All employees can work on their well-being with Thrive resources at any time throughout the year by logging into Virgin Pulse and selecting the Benefits tab from the top menu. Employees may also visit Thrive on the intranet, to access YNHHS well-being resources.

The following table includes the list of personal choice activities available to KYN Plus participants as part of Thrive. YNHHS benefits-eligible employees can earn YNHHS Medical Plan premium credit for these activities as indicated in the Credit Value column. Employees may participate in as many personal choice activities as desired, but the maximum credit they may earn is $500. Any credit earned by completing KYN personal choice activities will remain “locked” until completion of a health screening.

Use the Rewards tab in Virgin Pulse to find more detail on each personal choice activity and to track earned credits. Eligible activities completed from Oct. 1, 2021 through Sept. 15, 2022 can qualify for credit.

Dental Cleaning (up to two visits) * $50 each
Completion of a Virgin Pulse Health Risk Assessment – online health check $100
 Participation in Employee and Family Resources workshops and webinars (up to three classes) $100 each
Participation in a 5K or charity walk/run/bike (up to three events) or regular attendance at a gym (three days a week) *  $100 each
Volunteering at a non-profit 501-3 organization, such as Habit for Humanity(up to three events) * $100 each
YNHHS–sponsored financial wellness courses (up to three events) * $100 each

* These activities must be entered in the Virgin Pulse portal.

All KYN Plus activities must be completed by Sept. 15, 2022. Employees will receive a brochure in the mail to keep program details at hand.
