Jennifer Brackett, APRN, is an experienced family nurse practitioner and certified diabetes educator. Jennifer is passionate about helping patients in need of support with their diabetes, prediabetes or weight management. She provides education as well as medication management and routinely manages continuous glucose monitors. Jennifer earned her master's degree in nursing from the Massachusetts General Hospital (MGH) Institute of Health Professions and her Master of Public Health (MPH) degree from Boston University. She is currently seeing patients at our North Haven office by referral from their Northeast Medical Group primary care provider.
