Open Notes goes live October 2

Yale New Haven Health System and Northeast Medical Group are implementing OpenNotes, a national movement aimed at providing patients near real-time, secure access to their physicians’ notes in their electronic health record. The program, which appears as Shared Notes in patients’ MyChart portal, is scheduled to go live for all NEMG primary care physicians and specialists on October 2.

At the beginning of the Shared Notes launch, only notes from physician office visits – those covering the patient’s conditions and treatments – will be available. Other notes, such as procedure notes or hospital discharge summary notes, will not be available during this phase of the project.

All patients have a right to request their full medical record, including providers’ notes. Shared Notes helps to facilitate this process by giving patients access to their notes through the MyChart web portal. Notes currently cannot be viewed from the MyChart mobile app.
