CEO Message: November 2017

Dear Colleagues:

With Thanksgiving around the corner, I want to take the opportunity to thank you for the incredible care you provide to our patients and communities. This past fiscal year, we saw more than one million outpatient visits and over 300,000 inpatient visits. Additionally, across NEMG, we were ranked in the 94th percentile in our ambulatory patient engagement scores. Last month, we opened the first phase of our exciting Rye Brook Multispecialty Center and welcomed new physicians to the area.

As we expand and grow, we need to continually focus on Northeast Medical Group and YNHHS to provide high-quality, personalized care to our patients. This fiscal year, we have a renewed focus on enhancing the quality of care we provide our patients and achieving the "quadruple aim" of improving the patient experience, managing the total cost of care, improving the health of populations, and ensuring clinician satisfaction and engagement.

Our adoption of value-based care programs is one of the ways we are working towards achieving that goal. We currently manage over 114,000 lives participating in value-based programs through both commercial and public payors such as Medicare and Medicaid. 

To begin structured and thoughtful work in this area, we are looking beyond traditional clinic and hospital care. We are aiming to identify our patients' health through wellness visits performed by team members outside of traditional clinic visits. This helps us determine our patients' necessary preventive screenings and identify chronic conditions that require additional follow up.

This fiscal year, our focus is to close gaps of care where there is opportunity to do so. We plan to make enhancements in a number of areas, including our diabetic patients' care (particularly to eye complication screenings), follow up with our patients identified with depression and care and clinical continuity.

As we focus as a large medical group in providing population health, a concerted, coordinated effort from all of our teams and our patients becomes critical. To that end, Kathy Madden, our new director for population health; Arnold Dorosario, MD, our chief medical officer; Peter Logue, our VP of operations; and their teams are in the process of rolling out multiple work streams.
