Northeast Medical Group provides hospitalist services at Bridgeport, Greenwich and Yale New Haven Hospitals.

What are Hospitalists?

Hospitalists are specially trained physicians and advanced practice clinicians focused on treating the unique needs of patients during their hospital stay. Physicians are board-certified in Internal Medicine and treat a wide range of acute care and chronic medical conditions. Advanced practice clinicians include physician assistants (PAs) and nurse practitioners (NPs) supervised by physicians.

Northeast Medical Group Hospitalists

• Are available 24 hours a day, including weekends and holidays, to care for patients and answer any questions about their treatment plan.
• Provide regular visits, observe changes in your condition and serve as a knowledgeable resource for patients, families and staff.
• Work together with your primary care physician, as well as other hospital medical experts and specialists, to ensure the best inpatient care experience possible.
• Schedule tests and procedures promptly and follow up on results, as well as prescribe medications.
• Are part of an interdisciplinary team who work with physical therapists, case managers, respiratory therapists, dieticians, nursing staff and other healthcare professionals to ensure that the patient receives the expertise needed throughout their stay.
• Respond to emergencies and quickly address a patient’s changing medical needs.
• Offer consultations and evaluations for surgery, psychiatry, Ob/Gyn and the Emergency Department.

Who Takes Care of Me when I Leave the Hospital?

Northeast Medical Group hospitalists will speak to your primary care physician about your in-hospital diagnosis, treatment, any necessary follow-up care and provide notification of your discharge. If you do not have a primary care doctor, your hospitalist will refer you to one. Additionally, you can contact your hospitalist after discharge if you have any questions about your stay.
